OPT Updates Form

For students participating inĀ Optional Practical Training (OPT), please keep in mind that you must report changes in employment, U.S. address, name, and visa status within 10 days of the change.

Complete this form to update the O.I.A. with this information. If you change your visa status or the spelling of your name in your passport, please email the O.I.A. a copy of your new paperwork so that your immigration and Juilliard records can be updated. If you have any questions, email us at oia@juilliard.edu.

If you have departed the US (or will be departing within the next 15 days) and will not return to use your OPT, answer No.
Please email copies of your new immigration paperwork to OIA at oia@juilliard.edu so that OIA can end your F-1 record and update the Juilliard information system with your new status information.
If you depart the U.S. during your OPT and do not return to use the remainder of your employment authorization, keep proof of your departure for your records (i.e. flight itinerary or plane ticket).
If yes, please email a copy of your new passport to OIA at oia@juilliard.edu so that OIA can update your I-20.
