Once reviewed, the O.I.A. will provide you with a list of documents that must be submitted to our office prior to the visit, which may include a completed English Language Assessment. A copy of the rubric can be found here. If the O.I.A. determines that a J-1 visa is necessary, and the visitor is not a citizen or permanent resident of a country for which English is an official language, the rubric must be completed prior to the issuance of the DS-2019 (the form created by the O.I.A. and used by the visitor to apply for a J-1 visa).
If you have any questions, feel free to visit the Office of International Advisement in Suite 245, or email us at firstname.lastname@example.org, or call (212) 799-5000, ext. 358.
*If the visitor is a permanent resident/green card holder, STOP. You do not need to complete this form. Please work with the visitor, Human Resources, and Accounts Payable to complete appropriate paperwork.
Select all that apply.
If you are not sure, please consult with Human Resources as soon as possible as it may impact the visitor's visa options.
If the visitor is being provided a letter or contract, OIA will need a copy for the visitor's file. Please forward a copy to OIA once the letter or contact is finalized. Unsigned copies are acceptable.
